COSHH Form with instrument delivery
Please take note that instruments can only be accepted onto our premises with the legally mandated " Control of Substances Hazardous to Health Regulation" COSHH Declaration Form. This form should be clearly displayed so that the receiver opening the package is aware of it's contents and any potential risk.
